Takotsubo syndrome (TS) is a frequent complication of subarachnoid hemorrhage (SAH), especially in massive SAH with severe neurological damage. The initial presentation TS similar to acute coronary syndrome, causing differential diagnostic issues. Unnecessary steps and uncertainty therapy may delay the definitive treatment aneurysm, therefore increasing risk rebleeding. purpose this review summarize latest knowledge on diagnosis provide therapeutic algorithm for phase, promoting early aneurysm. Rapid hemodynamic stabilization aneurysm securing are key points reducing delayed cerebral ischemia improving outcomes. In noninvasive bedside methods preferred priority. combination electrocardiography, cardiac biomarkers, echocardiography great importance differentiating from myocardial infarction. risk-benefit ratio angiography should be carefully individually considered its use limited patients strong evidence ischemia, after successful endovascular Invasive monitoring beneficial cases cardiogenic shock or pulmonary edema. hemodynamical instability secondary TS, non-catecholamine inotropes, levosimendan recommended. refractory hypotension, mechanical support considered. left ventricular function improves within days months event, low ejection fraction predispose recovery.
